Physically it does nothing. Mentally, it tricks the min into believing pain is felt throughout the whole body, gnawing thru the marrow of every bone, as though they are in the center of a bonfire,w/flames on every side. Not a single cell in the body escapes. The pain is the worst imaginable pain burning inside the body (Being burned alive from the inside)
The ability is strong enough to incapacitate into a blubbering, withering heap on the floor.
The damage is sympathetic. like being beaten out of a possession suit.
